Dear Friends, The mission of the Spina Bifida Association of Delaware Valley (SBADV) is to promote the prevention of Spina Bifida and to enhance the lives of all affected. To be effective, we need a strong voice, and in numbers there is strength. Please complete and return the 2010 Registration Form (download PDF) to help us achieve our mission. Your registration will ensure that you continue to receive the SBADV Newsletter and are kept up-to-date on the most current information throughout our area.
SBADV provides programs and services to individuals living with Spina Bifida throughout the following counties: Philadelphia, Bucks, Montgomery, Chester, Delaware, Gloucester, Camden, and Burlington.
Additionally, anyone who wishes to receive the Spina Bifida Association’s national newsletter, “Insights”, can sign up for this newsletter through the SBADV registration form for a discounted price of $20.
The SBADV depends on fundraising and donations to support the numerous programs that are offered. If you would like to make an additional donation, we have provided an option on the registration form so you can help provide programs and services to individuals living with Spina Bifida.
Please return your registration form (download PDF) and fee at your earliest convenience to SBADV, P.O. Box 859, Worcester, PA 19490. All forms are due by December 1, 2009 to ensure inclusion in the 2010 Directory.
